Vertical Vector Extended Deep Plan Facelifting - Andrew Jacono, MD
Andrew A. Jacono, MD, performs the extended deep plane facelift he has developed with adjunctive procedures to rejuvenate the upper, middle and lower third of the face in this video. This facial rejuvenation procedure includes autologous fat grafting to the temple, periorbital region and midface as well as a modified lip lift and SMAS graft to the lips. A guiding principle in his technique is repositioning of deep facial to volumize the aging face and soft tissue sparing. The case contains a thorough review of the patient's preoperative appearance and surgical goals. Step-by-step procedure is demonstrated from incision to closure. Throughout the extended deep plane facelift procedure he explains the approach to the deep plane, release of the zygomatic, masseteric and mandibular cutaneous ligaments, elevation of the malar fat pad off the zygomaticus musculature, and vertical vectoring the deep plane flap to volumize the midface. The necklifting portion of the surgery highlights his technique of extending the deep plane dissection into the neck elevating an extensive platysma muscle flap inferiorly off the sternocleidomastoid muscle and medially. With a platysma myotomy, this flap is converted into a platysma sling supporting the submandibular gland and create a defined jawline contour. To further rejuvenate the midline neck his technique for opening the neck is described including sub-platysmal medial flap elevation, inter- and sub- platysmal fat removal, digastric treatment and hyoid fascia suspension of the midline platysma during corset platysmaplasty. -

Autologous Fat Grafting & Facial Rejuvenation - Thomas L. Tzikas, MD
Autologous fat grafting is a method of rejuvenating the face with soft tissue augmentation. The procedure is very artistic and technique dependent especially in knowing where to inject the fat and how much to inject in order to obtain natural and long term results. The harvesting and injection techniques should also be minimally traumatic. In this DVD, Dr. Tzikas steps the viewer through a pre-op analysis, fat harvesting and injection techniques. He clearly demonstrates areas for fat harvesting and fat injection. He explains the reasons for these choices. He discusses the amount of fat to inject in specific regions of the face. Pre and post operative photos are shown at the conclusion of the case. Saddle Nose Deformity: Composite Reconstruction - Rollin Daniel, MD
Saddle nose is one of the most challenging deformities in all of rhinoplasty surgery, especially when all structures are involved including a large septal perforation. A new approach, "composite reconstruction" is illustrated. Harvesting of rib cartilage is shown in step-by-step detail. The deeper "foundation layer" is constructed to provide support through the innovative use of spreader grafts and a true "septal" strut. The outer "aesthetic layer" consists of a columellar strut and tip graft plus diced cartilage in a fascia dorsal graft. Support of the collapsed alar rims is achieved with large lateral rim grafts. -

Deviated Nose with Dorsal Hump - Dean M. Toriumi, MD
In this DVD the surgeon will demonstrate his technique for management of the deviated nose. The patient has a deviated nose that is over-projected with a bulbous nasal tip. The external rhinoplasty approach will be described in detail as well as his execution of dorsal hump reduction, osteotomies, spreader grafts, lateral crural strut grafts, dome suturing, caudal extension graft, alar batten grafts and alar base reduction. This case demonstrates the philosophy of structural grafting and preservation of nasal support. Asian Blepharoplasty - Robert Goldberg, MD
In this DVD, Dr. Goldberg performs a closed blepharoplasty on an Asian patient. He demonstrates his technique for reforming an upper eyelid crease using a non-incisional mattress suture. There is no skin excision; sutures are passed in a form similar to the trail left by a downhill skier. The procedure is done under local anesthesia. Pre and postoperative photos are shown. -

A young, white female was desirous of a thinner, more defined nose with a take-down of her dorsal hump. She did not want too sharp of a tip. Standard open approach was utilized with suturing of the dome cartilages. Osteotomy techniques are highlighted with endonasal pictures. -
