The Endonasal Approach to Rhinoplasty has been used for decades. In the early years this approach was associated with a more reductive rhinoplasty and sequelae resulting in this becoming a less favored approach by those who are advocates for the Open Structure Approach which has its shortcomings. Dr. Williams’ presentation emphasizes the concept of Structural Rhinoplasty using the Endonasal Approach by employing grafting and suture techniques to accomplish the same goals with limited dissection and trauma to the nasal skeleton. Videos are included to show the details of the discussed and relevant surgical maneuvers so as to assist with understanding.

Dr. Gassner will also discuss the "S.O.F.T. Concept" in rhinoplasty, which was developed to minimize trauma, shorten recovery, maintain a naturally soft nasal tip, and preserve the skeletal and soft tissue elements of the nose. Representative cases will be shown to elucidate endonasal solutions to the entire spectrum of deformities and functional aspects of the nose.

Nasal tip aesthetics are complex and sublime. Correction of nasal tip deformities and creation of an attractive nasal tip is a challenge particularly along the alar lobule and soft triangle facet. This presentation will focus on surgical maneuvers to optimize both functional and cosmetic elements of this particularly challenging area of nasal tip anatomy.

Dr. Umang Mehta, MD covers a seven-step algorithm for a consistent, aesthetically pleasing and functional nasal tip in rhinoplasty. The steps include: Caudal Septal Extension/Replacement Grafts, Septocolumellar Suture Placement, Cephalic Trims, Cranial Tip Sutures, Lateral Crural Spanning Sutures, the Free Nasal SMAS Graft, and Premaxillary Grafts.

Now that we have had  6 months of experience following many of these guidelines and dealing with COVID, practitioners from around the country, we will discuss the measures they have adopted to practice safely in the office and operating room. Topics will include changes the office experience, virtual visits, exam room safety precautions, OR safety precautions, COVID testing, and new office protocols. We will also discuss the drawbacks and well as the benefits we have experienced with instituting many of these changes.

The incorporation of dorsal preservation into existing open structure rhinoplasty methods will be examined.  Variations in the methods will be explained and the methods will be reviewed with case examples and videos.  Outcomes measurements will be included.  Comparison to standard Joseph reduction will be reviewed.

Presenter: Richard D. Gentile, MD

Dr. Gentile has developed a new approach to the deep plane using Ultrasonic dissection that permits easy dissection of the facial deep plane and a complete deep plane and subplatysmal dissection that releases all posterior platysmal retaining ligaments. This technique has been used in close to 100 patients with excellent results and low complications. The technique is currently in press in the Facial Plastic Surgery Journal.
