The Endonasal Delivery Flap Approach to Primary Cosmetic Rhinoplasty - Stephen W. Perkins, MD
The endonasal delivery flap approach to a primary cosmetic rhinoplasty will be shown on a young 20-year-old female patient. The patient has some ethnic characteristics with slightly thick skin. She desired cosmetic improvement in the profile with hump reduction and refinement of the nasal tip. The preoperative analysis of her nose is presented by the surgeon, pointing out the anatomical and cosmetic issues shown on the preoperative photographs. The procedure begins with the endonasal delivery flap approach to the tip-alar cartilages. The caudal septal approach to the septum and harvesting cartilage is demonstrated. The "non" tongue-in-groove technique for managing cephalic tip rotation is shown clearly with management of tip projection. Sequential hump reduction followed by medial and lateral osteotomies is shown in detail. Fine tuning the result with nostril narrowing and a rim graft completes the procedure. Post- operative dressings are also shown. -

Vertical Vector Extended Deep Plan Facelifting - Andrew Jacono, MD
Andrew A. Jacono, MD, performs the extended deep plane facelift he has developed with adjunctive procedures to rejuvenate the upper, middle and lower third of the face in this video. This facial rejuvenation procedure includes autologous fat grafting to the temple, periorbital region and midface as well as a modified lip lift and SMAS graft to the lips. A guiding principle in his technique is repositioning of deep facial to volumize the aging face and soft tissue sparing. The case contains a thorough review of the patient's preoperative appearance and surgical goals. Step-by-step procedure is demonstrated from incision to closure. Throughout the extended deep plane facelift procedure he explains the approach to the deep plane, release of the zygomatic, masseteric and mandibular cutaneous ligaments, elevation of the malar fat pad off the zygomaticus musculature, and vertical vectoring the deep plane flap to volumize the midface. The necklifting portion of the surgery highlights his technique of extending the deep plane dissection into the neck elevating an extensive platysma muscle flap inferiorly off the sternocleidomastoid muscle and medially. With a platysma myotomy, this flap is converted into a platysma sling supporting the submandibular gland and create a defined jawline contour. To further rejuvenate the midline neck his technique for opening the neck is described including sub-platysmal medial flap elevation, inter- and sub- platysmal fat removal, digastric treatment and hyoid fascia suspension of the midline platysma during corset platysmaplasty. -

Endoscopic Forehead Lifting Using Multiple Myotomies for Excellent Long-Term Forehead/Brow Aesthetic Results - Stephen W. Perkins, MD
This nearly 35 minute video demonstrates how to endoscopically, consistently achieve long-term aesthetically pleasing forehead and brow lifting lasting 10 - 15 years and longer. Specific demonstration of incision placement, planes of elevation and extent and release of the arcus marginalis and conjoint tendon are shown and described in detail. Individual myotomies of the orbicularis, corrugators, procerus, and frontalis musculatures are effectively and efficiently performed endoscopically using a specially developed mono-polar electrical knife while safely preserving the main neurovascular bundles of the supraorbital and supratroclear nerves. Proper aesthetic elevation of the brows is achieved and fixated via cortical bone tunnels and bridges. -
