The Endonasal Approach to Rhinoplasty has been used for decades. In the early years this approach was associated with a more reductive rhinoplasty and sequelae resulting in this becoming a less favored approach by those who are advocates for the Open Structure Approach which has its shortcomings. Dr. Williams’ presentation emphasizes the concept of Structural Rhinoplasty using the Endonasal Approach by employing grafting and suture techniques to accomplish the same goals with limited dissection and trauma to the nasal skeleton. The COVID pandemic caused us all to rethink how to protect our staff, patients and ourselves from a virulent and somewhat unpredictable virus. During the 6+ weeks of lockdown, governmental agencies and medical societies produced suggested guidelines for safe reopening of practices.

Approaches to the deep plane have traditionally involved a Skoog type lower facial approach, a Hamra deep plane or a multi plane SMAS flap approach.

Presenter: Richard D. Gentile, MD
Dr. Gentile has developed a new approach to the deep plane using Ultrasonic dissection that permits easy dissection of the facial deep plane and a complete deep plane and subplatysmal dissection that releases all posterior platysmal retaining ligaments. This technique has been used in close to 100 patients with excellent results and low complications. The technique is currently in press in the Facial Plastic Surgery Journal.
